Durante o encontro, a profissional irá apresentar conceitos e compartilhar experiências relacionadas à implementação do PTS no CER ISD.
The Individual Therapeutic Project (ITP) is a set of interdisciplinary actions designed for an individual and their family, focused on needs centered on the health context. It seeks to overcome the traditional biomedical model, prioritizing the user's autonomy and the bond with the team. The process involves diagnosis, goals, division of responsibilities, and continuous reassessment.
